PURCHASE OF FORMS FOR POST-UTME SCREENING
This is to inform all prospective candidates for admission into Cross River State College of Nursing and Midwifery Sciences, Itigidi, that forms for Post UTME screening are available at the College Registry on payment of non-refundable fee of Twenty-Five Thousand, Five Hundred Naira (N25,500) only into Account No: 2037884057 (College Utility Account) domiciled in First Bank Plc and evidence of payment tendered at the Bursary department of the College where handling charges of ten thousand naira (N10,000) only shall be paid cash for issuance of receipts to cover all payments made.
(i). Procurement of form(s) commences on MONDAY 27TH MAY 2024.
(ii). Closing date for submission of forms is FRIDAY, 30TH AUGUST 2024.
ELIGIBILITY TO PURCHASE FORM(S)
1. Applicants who chose the College as first or second choice at JAMB Examination of 2024 with aggregate score of not less than 150.
2. Applicants with evidence(s) of change of institution/Course to reflect the name of the College with 2024 JAMB aggregate score of not less than 150.
3. Applicants for Post Basic Nursing and Post Basic Midwifery.
REQUIREMENTS TO OBTAIN FORMS
1. Applicants must present evidence(s) of five(5) credit passes in English Language, Mathematics, Chemistry, Physics and Biology obtained in WAEC or GCE, NECO or NABTEB O’Level at not more than two(2) sittings.
2. Evidence of 2024 JAMB score of not less than 150 and/or change of institution/Course reflecting the name of the College.
3. Two(2) recent passport-sized photographs preferably coloured with white background. First School Leaving Certificate and sworn Affidavit of date of birth/age declaration.
4. Evidence of RN or RM with Valid Licence from NMCN
Date for Screening Exercise:
Post-UTME screening shall commence Tuesday 24th to Thursday 26th of September 2024. The schedule for the screening exercise which shall comprise both Computer Based Test (CBT) and oral forms will be communicated to all and sundry at a later date for your earliest information and necessary action please.
